Hurricane Emergency Contacts

Did your public or private nonprofit facility sustain damage during Hurricane Ike?

If so, you may be eligible for public assistance. State government agencies, local governments and special districts, private nonprofit organizations that are open to the general public (including museums) and federally recognized Indian tribes should register with the Federal Emergency Management Agency (FEMA) for possible reimbursement. Please go to the Governor's Division of Emergency Management web site and select Disaster Recovery on the left menu. From the Disaster Recovery page, select Disaster Summary Outline and then submit the following forms:

* Request for Public Assistance
* Designation of Applicant's Agent
* Direct Deposit Authorization Form
* PNP Facility Questionnaire

Affected individuals and institutions must register for state assistance. FEMA’s filing deadline in Texas for Hurricane Ike is November 11, 2008. To see a list of declared counties, visit the Declared Counties page of the GDEM's web site.
