Substitute Reimbursement Request Form

Humanities Texas will reimburse schools up to $200 for a substitute for any teacher who attends one of our one-day workshops during the academic year. To receive reimbursement, please complete the following form to invoice Humanities Texas.

Multiple teachers may be included on the same form, but please complete a separate form for each workshop.
